Sample idea:

A great way to begin a story is to have the main character make a simple mistake. The consequences of this mistake are enormous and the character has to spend the rest of the story trying to either put things right or cover up what has happened. Of course things don't go smoothly and he finds himself in ever deeper levels of trouble until the end where... well, it's your story and frankly anything could have happened by then. Let's think of a few mistakes that the main character could make: misses an appointment, mistaken identity, adds the wrong ingredient to a mixture, presses the wrong button, miscalculates, says the wrong thing, doesn't listen properly, doesn't think before speaking, makes an assumption, misjudges the situation, forgets something important, fails to pass on a message, passes on the wrong message, passes on the message to the wrong person... You could keep adding to this list for ages, and each of them will make a great story.
